Yknow, when you think about it, having three heads is a bit more problematic because that also means three brains, and thus, at least in the cause of a cerberus type creature that is typically mammalian, that means three brains, which means triple the demand for oxygen, which means being in an oxygen deprived enviornment would probably make suffocation three times as quick unless the body was adapted to increase oxygen efficiency and capacity.
